Make a Referral

Our chapter serves children who live in Oregon and Clark County, Washington.  A child with a life-threatening medical condition who has reached the age of 2 1/2 and is under the age of 18 at the time of referral, is potentially eligible for a wish.

Qualifying for a Wish
There are three requirements for a child to qualify for a wish:

    1) The child must be referred when he/she is at least 2 1/2 years old, but before turning 18.
    2) The child’s physician determines whether the child is eligible for a wish, based on medical criteria provided by the Make-A-Wish Foundation® of America.
    3) The child cannot have received a wish from the Make-A-Wish Foundation® or any other wish-granting organization.

Referral Sources
Out of respect for the privacy of the children and families we serve, children who may be eligible to receive a wish can be referred by one of three sources:

    1) Medical professionals treating the child – such as doctors, nurses, social workers or child-life specialists
    2) A parent or legal guardian of the potential wish child
    3) The potential wish child

The Make-A-Wish Foundation respects the privacy of the children and families it serves. All medical information is considered confidential and is not discussed with outside parties unless it is required for the wish and the child’s parent(s) or guardian(s) have given their consent.
